Convert informal TODO and FIXME comments to Doxygen-recognized
      \todo format.

      - // TODO comments → //! \todo ...
      - // FIXME comments → //! \todo ...
      - /** @todo ... */ blocks → //! \todo ...
      - @brief TODO: placeholders → //! \todo (moved outside doc blocks)
